Establish a plan of care that includes your participation, customized to your specific needs
Communication and updates on medications, changes, side effects and pain levels
Communication and updates on your condition and progress
We encourage you to “Call Us first” rather than your primary care doctor when you have a question, a need, discomfort or a change in your condition. Working as a team, we want to make it easy to reach out.
An emergency plan is provided for you and/or your caregiver so you know what to do when emergent care is needed
